care coordination services

The Buckeye Ranch also offers Care Coordination services to youth and families who qualify for OhioRISE.

OhioRISE is different from the regular Ohio Medicaid plan for behavioral health because it brings together various providers and groups to make sure that children, youth, and families get the help they need. This happens through a care plan that values the input of children, youth, and families all guided by their strengths, interests, and needs based with a team approach.

To help families feel supported by this care plan, Care Coordination services are included. Children and youth with complex needs are often part of multiple community systems, like juvenile justice, child protection, education, and mental health. Thanks to OhioRISE coverage, Care Coordination connects these systems to create a personalized care plan for each individual’s behavioral health journey to ensure they have the right tools to heal.
